Skin Grafting on Wound with Exposed Bone

Introduction: In general, exposed bone wounds are covered with flaps. If the wound is small, skin grafting perifascial areolar tissue can be performed for resurfacing; however, it not suitable large bone-exposed exceeding 5 cm in length.Methods: Six patients underwent at our medical center between 2014 and 2020. all cases, a local flap donor site could found around wound, vessel to anastomose free was difficult find. After sequestrectomy, slit artificial dermis applied wounds, basic fibroblast growth factor sprayed. Ointment-impregnated gauze wounds. confirming creation of bed, performed.Results: after grafting, were closed. Four cases due extensive burns, one malignant tumor resection, injury. For burn injuries, donors Three exposure involved skull, rest anterior surface tibia. required split-thickness grafts (including two mesh grafts), full-thickness grafts. Covering took 3 weeks months (median, 1 month), which much longer than time taken when using flap. Conclusion: Even poor blood circulation, good bed created by combination bFGF, closure achieved grafting. Bone-exposed should closed early However, patient’s general condition or temporary unavailability donor, simple useful treatment option, although takes long time.
